Case Manager - Medix : Job Details

Job Summary Our client is seeking a Case Manager to conduct outreach and ensure the linkage of HIV positive cases to care. The role involves conducting educational seminars, client activities, supporting groups, and providing intern guidance. The Case Manager will participate in community meetings and represent the agency while ensuring timely service entry in the database. Responsibilities / Job Duties

• Conduct outreach and HIV testing; follow up to ensure positive cases are linked to care.
• Facilitate and/or organize educational seminars, client activities, and support groups.
• Provide guidance to interns and/or volunteers in service delivery.
• Participate in community meetings and represent the agency at events.
• Prepare monthly, quarterly, and annual reports for EIS, food, transportation, etc.
• Reconcile food and transportation data with delivery and productivity rates.
• Ensure timely and accurate service entry into the database.
• Link clients to resources through interagency/intra-agency referrals.
• Collaborate with clients to develop time-limited EIS plans addressing needs and barriers.
• Conduct monthly (or as-needed) client follow-ups to ensure EIS plan implementation.
• Identify eligible clients through intake and screening.
• Participate in delivery of food and transportation services.
• Assist clients/families with applications for community services and public benefits.
• Coordinate and network with other community resources to ensure service delivery.
