Case Manager - Garnet Health Medical Center : Job Details

The Case Manager actively coordinates the care of patients through a multi-disciplinary approach that focuses on the continuity of care from pre-hospital admission to post-hospital discharge. The Case Manager, completes an initial assessment of patients physical, cognitive, living arrangements, supportive system status and initial discharge plan. The Case Manager coordinates discharge planning activities (i.e. Homecare Services, DME, Rehab, SNF, OP Services) with active involvement of patient, patient representative, Medical Staff, Nursing Staff, and other disciplines to ensure that the most appropriate discharge plan is implemented for patients continuum of care. The Case Manager identifies patients psychosocial & socioeconomic issues that may interfere with the patients continuum of care progression as well as provides supportive counseling & community referrals (i.e. counseling services, DSS, APS, and CPS) as necessary.
