ESSENTIAL FUNCTIONSTo help ensure that each participant remain stable at the site and make progress toward his permanency goals prescribed by his permanency order. This position will be responsible for performing day-to-day support services assistance to the participants of the program. To manage all aspects of a participant's case (Court visits and court reports, ACR reports and visits, participant advocating, medical/dental visits, hospital visits, family attachment, etc.) as it relates to their residence and growth while at Garden of Prayer Youth Center.ORGANIZING AND COORDINATINGScreening/IntakeConduct intake process and intake meetings with therapist, DCFS worker, and participant.Assure the retrieval of necessary documentation for participant admission into the program.Conduct participant orientations to familiarize them with program expectations, conduct clothing inventory, and build rapport.Conduct the process of receiving new participants.Ensure 906 is completed promptly.Some duties may be reassigned and other duties may be asked of you as required. Interfacing - CommunicationMaintain proactive, effective communication with participants, direct care staff, therapist, and Program Coordinator regarding all participant issues and concerns.Maintain proactive, effective communication with participants, participant family, DCFS workers, DCFS Monitor, DCFS Licensing Monitor, Program Coordinator, GAL, and all other stakeholders involved with the participant's case.Ability to observe participant behavior and participation, assess appropriateness of such, and prepare reports and other correspondence in order to advocate and communicate all pertinent information regarding participants.Build positive rapport with participants, colleagues, all DCFS personnel, and all other stakeholders involved with participant's case.Set a good example for participants including cleanliness, punctuality, sportsmanship etc...Maintain positive public relations with participant's families.Facilitate interaction between participant, and participant as well as participant, and staff.Participate in department and company employee meetings.Provide frequent feedback and positive reinforcement to participants.Interact and effectively communicate with colleagues in a professional fashion.Communicate all necessary information to Lead Staff and Program Director.Some duties may be reassigned and other duties may be asked of you as required.Linkage Serve as participant's advocate to court system, school system, etc.Manage all participants' medical/dental, psychiatric, appointment and ensure they remain current.Manage all participants' ACRs, court visits, school enrollment.Some duties may be reassigned and other duties may be asked of you as required.Monitoring Provide added supervision of participants to milieu. Monitor all case management issues for participants and ensure that all documentation and visits are current.Monitor participant campus attendance.Monitor and maintain participant visitation plans and procedure.Monitor the procurement of drug screening of participants by staff.Some duties may be reassigned and other duties may be asked of you as required. Direct ServiceAttendance at all scheduled court and Administrative case reviewsCompleting the 497 (service plan) according to DCFS policy and ensuring that the youth is in attendance at the case reviewAssures that the youth attends the scheduled CAYIT for purposes of planning, when necessaryMonitors the youth progress, academically. This includes but is not limited to school/vocational enrollment, collecting and maintaining school records and documents, collaborating with school staff and attending school conferences or staffingsMaintain client case file in AP-5 orderComplete and/or make sure that UIR's are written, distributed and maintainedAssure that the client has access to and meets with GAL and all legal representationAssure that all court, probation or parole orders are followed and adhered toArrange sibling or parental visits if appropriate and upon request and document all visitsAssure youth is at all court appearances when attendance is mandatoryWeekly visitation to youth when hospitalized or incarcerated and provide youth with allowanceContacting SASS for an evaluation/screening in the event that a youth is psychiatrically hospitalizedObtain all necessary consentsUnusual Incident ReportingAll other duties as assigned according to DCFS policy Some duties may be reassigned and other duties may be asked of you as required. ADMINISTRATIVE Documentation/Record KeepingPrepare and submit all documentation and reports accurately in a grammatically correct fashion.Documentation of any Unusual Incident of any participants on a UIR Form.Submit all necessary documentation to Program Coordinator, HR Dept., DCFS personnel and Executive Director as requiredSome duties may be reassigned and other duties may be asked of you as required.Agency Activities/Training/OtherAttend and participate in all initial and on going trainingActively participate in orientation and staff training, assigned staff meetingsAttend All Staff meetingsRequirementsQUALIFICATIONSEducation: Requires a bachelor's degree in social work or a related human services field from an accredited college or universityExperience: 3 years experience working with at-risk youth.
