Qualifications Non-Medical Case Manager (Louisiana)
Education & Experience
- High school diploma or GED required; Associate's or Bachelor's degree in Human Services, Social Work, Psychology, or related field preferred.
- Minimum 12 years of experience working with individuals with developmental disabilities, the elderly, or other vulnerable populations.
- Familiarity with Louisiana Medicaid Waiver Programs, including CCW (Community Choices Waiver), ROW (Residential Options Waiver), SIL (Supported Independent Living), and other HCBS (Home and Community-Based Services) programs.
Knowledge, Skills, & Abilities
- Understanding of ADLs (Activities of Daily Living) and IDLs (Instrumental Activities of Daily Living) support requirements.
- Ability to create, review, and implement individualized service plans (ISPs).
- Basic knowledge of medication administration guidelines and ability to verify medication training compliance for Direct Service Workers (DSWs).
- Strong organizational, documentation, and time management skills.
- Proficient in Microsoft Office Suite, case management software, and electronic health record systems.
- Excellent written and verbal communication skills, including the ability to work with clients, families, providers, and state agencies.
- Conflict resolution and problem-solving skills.
Licenses & Requirements
- Current valid driver's license.
- Reliable transportation with proof of insurance (travel between client homes and community resources required).
- Ability to pass state-required background checks and drug screening.
- Completion of state-required trainings, including Abuse/Neglect Prevention, HIPAA, Person-Centered Planning, and Crisis Intervention.
- CPR and First Aid certification preferred.
Physical Requirements
- Ability to sit, stand, and walk for extended periods.
- Occasional lifting of up to 25 pounds to assist with mobility aids or daily activities.
