Job DescriptionJob DescriptionSeeking a Case Manager to provide client-centered case management to the Populations of Focus. The Case Manager will also engage in outreach and program promotion functions. The Program provides care and case management services offered by a Case Manager to older adults who may be frail, low-income, and adults 18 or older who may be functionally impaired to avoid and/or delay placement in a nursing facility. Pending eligibility, the Program serves as an alternative care and case management option for older adults or adults with functional impairment who may not be eligible for the Client Multipurpose Senior Services Program or CalAim Enhanced Care Management services. CalAIM Enhanced Case Management provides client-centered case management and chronic condition management and chronic condition support to frail, older adults at risk for institutionalization, eligible for long-term care, nursing facility residents who desire to return to living in the community, and adults 18+ and high utilizers of health services with the program goals of decreasing hospitalizations and preventing institutionalization. In addition to direct client services, this role will play an integral part in supporting program growth through focused outreach and promotion efforts. ECM encompasses seven core services: 1. Outreach and Engagement 2. Comprehensive Assessment and Care Management Plan 3. Enhanced Coordination of Care 4. Health Promotion 5. Comprehensive Transitional Care 6. Member and Family Supports 7. Coordination of and Referral to Community and Social Support Services Specific position responsibilities:• Collaborate with internal teams and community partners to develop and implement outreach strategies that increase program awareness and referrals.• Represent Client at community events, health fairs, and partner meetings to promote services and build referral pipelines.• Track and report outreach activities and referral trends to inform program development and scaling efforts.• Support the development of outreach materials and messaging that align with program goals and target populations.• Conduct timely assessments for incoming and existing members assigned to ensure that all necessary information is collected to develop an actionable plan.• Prepare and maintain comprehensive member files via the reporting system.• Engage in community outreach to inform potentially eligible individuals on the availability of services, programs and resources available within Client and network.• Ensure accurate data entry and management of electronic client files are maintained.• Participate in data audits and required monitoring of client files, as needed with timely response to correct outstanding errors to ensure organizational compliance.
