As a Case Manager you will coordinates and oversee patient care, ensuring timely and cost-effective treatment. Education and/or Experience Qualifications:

• A Bachelor's Degree in a Human Services field from an accredited college or university.
• Minimum of two (2) years of relevant work experience.

• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Flexibility and adaptability in a fast-paced environment.
