Purpose:
- Provide case management services for the clients of The Salvation Army Harbor Light
Summary:
- Facilitate didactic groups and case management services
Primary Duties and Responsibilities:
- Conducts didactics/educational lectures
- Maintain case records in accordance with Harbor Light standards
- Serve as Case Manager to treatment clients
- Assists clinical staff with various tasks as assigned (i.e. make packets, assist clients with nonclinical issues, etc.)
- Participates in the review of all clinical issues, activities and program policies and procedures
- Attends all clinical staff meetings, and team conferences
- Participates in educational courses, seminars, workshops, current literature and in-service meetings to enhance treatment skills
Qualifications:
- Ability to determine needs and to work with multi-problem individuals
- Ability to develop rapport, trust and confidence in client relationships
- Must possess good communication and teamwork skills
- Strong organizational skills
- TB Test and police clearance required
- Authorization to work within The United States
Education/Experience:
- A bachelor's degree in social work or an equivalent comparable human services field is required
- Experience in case management preferred
- Experience with mental health and/or substance abuse populations preferred
Computer Skills:
- Excellent computer skills required
- Experience using electronic medical records systems preferred
Reasoning Ability:
- Understand and work within The Salvation Army and Harbor Light Mission Statements
- Possess an excellent working knowledge of The Harbor Light Policies and Procedures
Certificates and Licenses:
- Must possess MCBAP credential (CADC, CADC-M, CAADC) or have an approved Development Plan within 2 weeks of hire date
- Valid Michigan Chauffeur License and approved by The Salvation Army MVR
