Title: Case ManagerSchedule: Full Time Salary: $60,450 - $63,000/year - based on experience Benefits:Generous PTOPaid Holidays and Floating HolidaysBlue Cross/Blue Shield Health Plans, Dental, VisionChild Care ReimbursementCompassionate PTO Donation ProgramCOVID-19 Vaccinated WorkplaceFlexible SchedulingEmployer Paid Life InsuranceEmployee Assistance ProgramPTO Cash Out ProgramRetirement Plan - 401(k) with company matchWe are currently seeking a dedicated and passionate Case Manager with experience in Senior Living.Case Manager Responsibilities include: •Orient and assist each resident to adjust to life in the community •Ensure or provide therapeutic interventions to assist residents in coping with their transition and adjustment to facility, including their social, emotional, and psychological needs •Assist each resident to maintain family and community ties and to develop new ones •Complete quality management reviews to ascertain if the department is meeting state regulations and PSL policies and follow up as needed •Work with the care department and others to promote and protect resident rights and the psychological well-being of each resident Case Manager Education & Experience: •Bachelors degree with major course work in human services AND one year of full-time work experience in a related field •Case management experience required•Memory Care/ Assisted Living experience required•Compassion for and desire to work with the elderly Company Overview: Peregrine Senior Living is known for their innovative leadership in revolutionizing care for the elderly.
