Provide case management services for the clients of The Salvation Army Harbor Light.Essential Responsibilities:Conducts didactics/educational lecturesMaintain case records in accordance with Harbor Light standardsServe as Case Manager to treatment clientsAssists clinical staff with various tasks as assigned (i.e. make packets, assist clients with nonclinical issues, etc.)Participates in the review of all clinical issues, activities and program policies and proceduresAttends all clinical staff meetings, and team conferencesParticipates in educational courses, seminars, workshops, current literature and in-service meetings to enhance treatment skillsQualifications:Ability to determine needs and to work with multi-problem individualsAbility to develop rapport, trust and confidence in client relationshipsMust possess good communication and teamwork skillsStrong organizational skillsTB Test and police clearance requiredAuthorization to work within The United StatesEducation/Experience: A bachelor's degree in social work or an equivalent comparable human services field is requiredExperience in case management preferredExperience with mental health and/or substance abuse populations preferredComputer Skills: Excellent computer skills requiredExperience using electronic medical records systems preferredCertificates and Licenses: Must possess MCBAP credential (CADC, CADC-M, CAADC) or have an approved Development Plan within 2 weeks of hire dateValid Michigan Chauffeur License and approved by The Salvation Army MVRPhysical Demands: Ability to lift 10 poundsFrequent sitting for long periods of timeCapability to inspect packages, equipment, and fixtures, as well as observe people and eventsThe ability to hear and understand information and ideas presented through spoken words and sentencesCapability to use standard office machines like copiers and fax machinesIn addition: Must be highly organized, motivated, a self starter with the ability to multi-task, be creative.
