We are TASC!Position Overview: TASC is seeking a dedicated Case Manager for our Corrections Transition Programs (CTP) division located at Sheridan Correctional Center. This position plays a vital role in supporting individuals returning to the community after incarceration by offering specialized reentry case management. The ideal candidate will provide addiction intervention, AODA counseling, and recovery support services to clients and their families while coordinating access to critical resources.Please note: Candidates must be comfortable working inside a prison environment.Key Responsibilities: Conduct comprehensive assessments and develop individualized service plans Facilitate intake, enrollment, and coordination of services inside correctional facilities and in the community Provide referrals and crisis intervention, including behavioral health, housing, public benefits, and employment Collaborate with internal teams and external partners to ensure holistic and integrated care Monitor and document client progress toward service goals, using systems like ASAM criteria and DSM-5 Complete accurate, timely records including progress notes, service plans, and discharge summaries Assist clients in obtaining vital records and enrolling in public benefit systems Attend team meetings, staffings, and community partner events to support client careOur Ideal candidate: - High school diploma or GED equivalent Valid driver's license CADC certification within two years of employment with TASC may be required based on contractual obligations Must be able to pass an IDOC Background check Demonstrated proficiency in the Microsoft Office suite Demonstrated experience in group facilitation Knowledge of the current ASAM placement criteria, DSM-V as it relates to substance-related disorders, and Code of Ethics for substance abuse professionals, or the ability to acquire the knowledge Comfort working inside a correctional setting and with persons convicted of serious violent offenses Flexibility in thinking and approaches to leading group discussions Good verbal, written, and interpersonal communication skills Good time management skills with a proven ability to meet deadlinesBenefits Include: Medical, Dental, Vision, and Life Insurance Flexible Spending Account 403(b) Retirement Plan Tuition Reimbursement Paid Time Off, Sick Leave, and Floating HolidaysReady to Make an Impact?Apply online at www.tasc.orgTASC is an Equal Opportunity Employer and a Drug-Free Workplace.
