Case Manager Bilingual Spanish A PLUS

Schedule: MONDAY TO FRIDAY 9AM TO 5PM

$25 hour

What We Do:

Our client believes everyone deserves a home. We work to ensure that tenants have the tools and support they need to achieve housing stability and long-term recovery.

What You'll Do:

• Conduct intake and needs assessments for tenants and families, with periodic re-assessments.
• Review documentation to establish tenant eligibility and maintain accurate tenant files.
• Develop independent living plans with tenants, setting both short- and long-term goals.
• Help tenants access community resources and make referrals to internal and external services.
• Support tenants in overcoming barriers, advocating for themselves, and working toward self-sufficiency.
• Monitor and document tenant progress through regular contact and visits.
• Participate in case conferences and follow up with tenants and referral organizations.
• Assist tenants with applications for benefits and entitlements; may escort tenants to appointments.
• Conduct home visits to evaluate living conditions and provide continued support.
• Input data into the program database and prepare required reports (weekly, monthly, quarterly, and annual).

What We're Looking For:

• Bachelor's degree (B.A., B.S.W., or related field).
• Strong advocacy, communication, and problem-solving skills.
• Compassion, patience, and a commitment to helping clients achieve long-term stability.
