SUMMARY:
Create and maintain a sense of community among participants, family, staff and co-workers. Focus on enhancing participants' self-esteem and independence through opportunities for choice and decision making. Maximize opportunities for community integration. Provide outstanding support and service that will increase the individuals' self-sufficiency and independence. Serve as a role model for others through positive and respectful interactions with all members of our community. Listen and observe. Seek out and pay attention to the preferences and interests of participants. Complies with all principles of Human Rights, D.P.P.C., Department of Developmental Services, the Division of Medical Assistance and all other applicable funding regulations as adhered to by Lifeworks.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Provide individual and small group instruction in self-care, activities of daily living, communication, social and motor skills.
Provide direct care for ADLs and group activities. Lift and transfer individuals as appropriate to their needs.
Coordinate services with Interdisciplinary Team.
Communicate respectfully and pro-actively with families, residential staff and all members of participants' teams.
Develop and implement Day Habilitation Service Plans (DHSP), including:
Participate in scheduled supervision sessions with Assistant Director.
Participate in program-wide staff meetings and case review meetings as scheduled.
Participate in team meetings regarding general program development, including curriculum development and Schedules.
Ability to demonstrate good computer skills as well ability to learn to utilize the agency's computer programs for documentation of necessary data.
Attend in-service trainings as required. Ability to work in a team environment.
Ability to multitask and prioritize tasks.
Strong written and verbal communication skills along with excellent interpersonal skills.
Comply with all applicable State and Federal regulations, agency policies and procedures, and personnel practices.
Protect the health and safety of all participants and staff.
Performs other duties as requested by supervisor(s).
EDUCATION and/or EXPERIENCE BA/BS in Human Services /related field or associate degree plus 3 years related experience with persons with ID/DD or Human Services or HS Diploma/GED plus 5 years of related experience with persons with ID/DD or human services.
