Job Type Full-timeDescriptionThe Case Manager (CM) is responsible for managing, planning, organizing, and marketing the three Albany housing programs and their services under the Director of Housing. The Case Manager ensures that program objectives are met and supervises program staff. He/she assists residents in obtaining necessary entitlements, health services, support services, and access to other service providers as identified by their individual service plan. He/she solicits referrals from community, state and local facilities and other providers. RequirementsESSENTIAL RESPONSIBILITIES
- Ensures programs are operating at maximum occupancy.
- Coordinates incoming referrals.
- Networks and communicates with referral sources to ensure resident needs and goals are met.
- Schedules and co-facilitates house meetings.
- Provides community-based outreach and education.
- Responsible for the provision of residents' security.
- Schedules program staff, supervises and evaluates staff, and maintains communication with staff.
- Responsible for monitoring, reviewing, and completing documents, forms, and reports required by licensing agencies and funding sources relevant to the housing programs.
- Maintain a workplace environment that will attract, retain, and motivate people, both professional and volunteer.
- Participates in service-related community planning and outreach meetings.
- Meets regularly with each resident to discuss issues, concerns, goals, progress, and obstacles.
- Assists in the development and implementation of a program of quality assurance for resident services.
- Ensures that the Associate Director of Housing is informed of the condition of the programs and services, and all important functions influencing them.
- Performs other duties as assigned by the Director of Housing.
EXPERIENCE
- 2+ years of experience within social work, case management, or human services.
- Possess experience working with diverse populations and individuals who are: living in poverty, have a history of homelessness, are experiencing mental health challenges and/or substance use or addiction, have prior incarcerations, and/or physical, mental, or developmental disabilities.
EDUCATION
- Bachelor's degree in human services, or a related field, and/or at least three (3) years' experience in a not-for-profit agency, or any combination thereof.

WORK WEEK Full-time role, 40 hours per week.
