Located on the Upper East Side, this 23-story, 215-unit residence offers assisted living, enhanced care and memory care. This is an exciting opportunity to be a part of a brand-new development, the first of its kind in New York City. This role will be a critical part of the team in preparation for opening.

The Care Manager (Case Manager) Role: Responsible for providing care management and service coordination for residents in a combined assisted living, memory care and enhanced care environment under the Senior Director of Integrated Care.

  • Ensures accurate, timely, and professional delivery of care and services to all residents and is accountable for knowing state regulations related to the provision of services to ALR/EALR/SNALR residents and maintaining regulatory compliance.
  • Provides coordination of care between the resident, family, PCP, specialists, acute and post-acute providers, and other members of the interdisciplinary team.
  • Performs compressive assessments on residents at regular intervals and as needed so that the ISP is always up to date.
Care Manager (Case Manager) Education/Experience/Licensure/Requirements:
  • Minimum of 1 year of care management experience with experience in hospital, senior living, LTC, SNF, and/or home health care
  • Minimum 2 years working with older adult population with complex medical needs including dementia
  • Excellent written and oral communication skills
  • Strong clinical skills; Exceptional assessment and care management skills
  • Excellent independent judgement and decision making
  • Proficient with MS Word and Excel as well as electronic health records
  • Knowledge of state and federal regulations and the survey process
  • Thorough understanding of geriatric care as well as Alzheimer's disease and related dementia's
  • Demonstrates ability to effectively communicate with team members, family members, and external providers
  • Able to present effectively in front of groups
  • Possess and demonstrate a sincere passion for working with the older adult population
  • Creative, energetic, enthusiastic, patient, flexible, encouraging, and team-oriented
  • Demonstrates ability to organize time with efficiency and effectiveness
  • Demonstrates ability to appropriately prioritize situations based on level of acuity or importance
  • Proficiency conducting and documenting a complete psycho-social assessment
  • Demonstrates knowledge of and ability to use screening/assessment tools
  • Proficiency with SBAR Communication
  • Able to use on-line training platforms, and complete and update a Care Plan
  • Able to lead case discussions on residents with complex care needs and effectively lead family/team meetings
  • Must be accessible by phone 24 hours a day when not working to address care management concerns
  • Required to work extended hours and to report to work during times of need including inclement weather and when building concerns arise
  • Ability to stand for long hours and walk long distances without fatigue
  • Ability to bend, stretch, stoop and reach repeatedly without difficulty; Light work Exerting up to 60 pounds of force occasionally, and/or up to 10 pounds of force frequently or constantly to lift, carry, push, pull, or otherwise move objects
  • Criminal background check required
