Benefits Salary: 48,000 to $53,000 For Full Time Employees Benefits/Perks: Tuition Assistance Program Generous paid time off (Including 5 Self-Care Days/Floating Holidays, 12 Sick Days, 15 Vacation Days, and 11 Holidays) Healthcare Plans (Medical, Dental, Vision, and Pet Care) 403(b) Plan (GSS contributes 3% of your salary to your 403(b) plan after 3 years of service, with contributions increasing over time) Candidates hired in this position will work in person but will have the flexibility to work a hybrid schedule when program need allows Overview As a member of a multidisciplinary team, the Time Limited Rental Assistance Program Case Manager is responsible for providing a full array of counseling, case management, advocacy, and group work services to young adults ages 18-24 experiencing homelessness who are preparing to transition to permanent housing in the community, while gaining job training, employment, and educational and life-skills competencies. Reports to: TLRA Program Director Primary Location: 1995 Jerome Avenue Bronx, NY 10453 (& NYC/Citywide Area) Hours: 35 Hours/Week; Non-Exempt Salary: 48,000 to $53,000 Major Duties: Oversee the diverse case management needs of approximately 10 young people experiencing homelessness funded through DYCD/RHY and HUD. Provide direct service, including individual counseling to program participants a minimum of twice a month. Conduct biopsychosocial assessments with program participants to better understand their strengths, needs, challenges, and potential risks. Work with program participants to develop and implement individualized action plans and goals based upon their specific service needs. Using evidence based tools, assess history of housing and homelessness, risks faced, socialization, daily functioning, and health and wellness. Participate in the program participant referral process which can include identifying and interviewing potential participants, as well as orienting Identify appropriate resources to assist young people on their journey to living independently. Responsible for coaching program participants develop healthy and appropriate recreational and social skills, budget management, health management, etc. Be available to escort program participants to appointments if needed. Assist program participants in managing and resolving potential crises. Provide advocacy and assistance in making referrals to entitlements, legal assistance, providers for educational, vocational, medical, and mental health services as needed. Responsible for documenting services/contacts on the GSS program database and other such city databases, as well as additional specific documentation in a timely manner. Responsible for collaborating/communicating with program staff and staff from external organizations in which the program participants participate in or are referred to. Participate in all meetings and trainings as directed. Represent the program and organization in external meetings that would provide resources, opportunities to network with other providers, etc. Responsible for providing aftercare services on a quarterly basis to all program participants who have managed to complete the full three years of the program. Promote and model the 7 Sanctuary Model commitments and the SELF (Safety, Emotional Management, Loss, Future) Model into daily practice to create a non-violent and trauma informed culture Other duties as specified. Qualifications: Bachelor's Degree Required. Two years of successful experience providing services to at-risk youth (including LGBTQ) in areas of assessments, individual and group counseling and development and implementation of ISPs. History of successful collaboration with other community based agencies and organizations to enhance services for vulnerable youth, as well as experience working with City systems. Flexible hours to insure access and availability. Must have a clear understanding of the needs of young adults experiencing homelessness and a sensitivity to ethnic and cultural differences, and an understanding of the needs of the LGBTQ community. Must have a commitment to work from a strength based, youth and family development and trauma informed perspective. Excellent crisis intervention, advocacy and problem solving skills. Must be able to communicate clearly and advocate on behalf of program participants. Must have excellent written and verbal communication skills to maintain required documentation.
