Application Deadline: 9/12/2025 About the Role: Family Supportive Housing is committed to creating, designing, and implementing community services from a proactive, holistic, and person-centered approach. As a valued team member, you will provide culturally responsive, trauma-informed case management services and referrals to community resources to remove barriers to maintaining or stabilizing housing. Complete daily documentation, interpret reports/assessments from providers, and maintain complete and accurate client records. This is an in-office position with potential to transition to hybrid after demonstrating competency and completing training.Qualifications: An Associate degree in Social Work, Psychology, Sociology, Human Services, or a related field is required, along with at least two years of relevant experience. A Bachelor's degree in a Human Services field is preferred, accompanied by a minimum of one year of related work experience. However, strongly relevant work experience of at least four years may substitute for the Bachelor's degree preference. A solid understanding of homelessness, mental health issues, substance abuse, navigation of housing resources (including eviction prevention), domestic violence, and community resources is preferred. Candidates must possess a valid Minnesota driver's license and proof of insurance.
