Your Role as a Case Manager

As a Case Manager, you'll:

  • Collaborate with an interdisciplinary team, guiding patient-centered treatment plans.
  • Coordinate and implement discharge planning that meets patient needs.
  • Monitor patient experience to ensure quality and timely care.
  • Facilitate weekly team conferences to adjust treatment plans as needed.
  • Document case management addendums and ensure compliance with regulations.
  • Engage with patients and their families, providing education about rehabilitation.
  • Assist in securing resources that prevent discharge delays.
  • Participate in the utilization review process, understanding key indicators and making improvements.
  • Be available for on-call duties as necessary.

Qualifications

  • License or Certification:
    • Must hold an active license in your discipline (RN, SW, OT, PT, ST, or Rehabilitation Counseling).
    • CCM® or ACM certification is required or must be obtained within two years of entering the position.
  • Minimum Qualifications:
    • For Nursing, an Associate Degree in Nursing is required, with a BSN preferred.
    • For other health care professionals, a minimum of a bachelor's degree is necessary; a graduate degree is preferred.
    • Preferred: 2 years of rehabilitation experience.
