Job DetailsJob LocationBrooklyn, NYPosition TypeFull TimeSalary Range$55000.00 - $55000.00 Salary/yearDescriptionPOSITION: Case ManagerREPORTS TO: Director of Clinton Avenue ResidenceCLASSIFICATION: Full TimeEXEMPT/NONEXEMPT: Salaried Non-ExemptGENERAL DESCRIPTION:The Case Manager is responsible for providing case management services to the male juveniles detained by the department of juvenile justice for non-secure detention within the Clinton Avenue Residence.Case Managers are classified as essential workers, and as such accept responsibility for the completion of their critical services to youth, on site, even during times of emergency. These include but are not limited to emergencies caused by weather, nationwide pandemics, civil unrest and domestic threat. MAJOR DUTIES AND RESPONSIBILITIES:Represents Lutheran Social Services of New York's mission, values, ethics, and uphold agency Code of Conduct at all times.Maintain regular contact with residents and families in accordance with OCFS and DYFJ mandates.Document all contacts on progress notes. Complete adjustment reports as ordered by the family court as needed. Monitor court documents to ensure court orders are fulfilled.Facilitate group and/or rap sessions with residents and create monthly group session calendar.Overseeing and conducting intake interviews with potential residents for vacant unit/housing and complete all required documentation. Coordinate visitation and phone call nightsEnsure behavior management system is adhered to agency policies. Maintain resident files.Advocate for residents and facilitate case conferences as needed.Refer residents for mental health services as needed.Ensure follow-ups for all referrals and services.Track appointments and coordinate with staff to ensure compliance.Attend DYFJ case manager meetings as scheduled.Report any emergency situations and pertinent information to the Director immediately. Perform any other tasks assigned by the Director.Adhere to confidentiality protocols outlined by the agency.Demonstrates commitment to social justice and promotes equity by identifying and removing barriers, facilitating learning, cultivating community, and empowering others.SUPERVISES:Volunteers (if applicable)QualificationsQUALIFICATIONSEDUCATION & EXPERIENCE:Master's degree preferred.Minimum Bachelor's Degree with three years of case work experienceExperience working with at-risk youth and family preferred.SKILLS & ABILITIES:Excellent written and verbal communication skills Knowledge of Microsoft Office applicationsKnowledge of behavior management and child developmentAssess the needs of individual youth and refer for appropriate services.Ability to manage time, organize and multi-task. Ability to work well with at-risk youth and their families.Ability to maintain a safe and controlled environment.Ability to apply training to work environment.OTHER REQUIREMENTS:Valid NYS driver's license DATE REVISED: 8/30/23
