The KKV Weekend Case Manager/Community Support Peer provides case management and recovery support to community members of KKV from Friday through Monday, including weekends. The KKV Case Manager will work within a multi-disciplinary team to assist unhoused individuals suffering from substance use and mental health issues. Responsibilities include transportation, targeted outreach, engagement with individuals experiencing co-occurring disorders, and addressing housing disparities. The incumbent will deliver case management and peer support recovery coaching services under the supervision of the KKV Social Worker and Program Director. These services encompass coordinating community support services, aiding stabilization after homelessness, providing prevention services, promoting self-advocacy, developing natural support networks, and fostering community living skills. The focus is on ensuring the stability of KKV clients and supporting team members within the KKV community.
