This is responsible and specialized case management work in assessing, planning, linking, monitoring, and advocating for a caseload of children/adolescents with serious and persistent mental illness. The incumbent is responsible for assisting clients with their needs/goals through the development of the treatment/service plan and/or aftercare plan. Work also entails responsibility for working with clients' families or guardians and community resources and advocating for clients by linking them with appropriate services. Education/ Training:
- A bachelor's degree in the field is required.
- At least one year of experience working with behaviorally challenging children or children with co-occurring disorders.
Job Duties: - Uses their own family experience as a tool to model for families - Increases family's knowledge and skills by utilizing evidence-based parenting curriculum when necessary - Provides information about social, emotional, behavioral, and/or substance use-related resources - Assists in supporting families in crisis - Facilitates family-directed services, goal setting, and communicating effectively with other treatment providers using the wrap-around approach -Assists in advocacy within the community - Provides practical support to caregivers in regard to day-to-day issues occurring within the home as they relate to overall family well-being
