Are you passionate about helping others, collaborating with a mission-driven team, and creating lasting impact in your community? Join Queens Community House (QCH) as a Case Manager in our Case Management Program, where you'll provide compassionate, personalized support to older adults in Queens.

QCH is a multi-site, community-based nonprofit serving the diverse population of Queens through a broad network of programs. Rooted in the settlement house tradition and driven by a modern approach to social and economic justice, QCH equips individuals and families with the tools to enrich their lives and strengthen their communities.

About the Role

As a Case Manager, you will support approximately 70 older adults living in Community Districts 4 and 6—including Elmhurst, Corona, Forest Hills, and Rego Park—as well as Jamaica, Queens, in partnership with Catholic Charities Neighborhood Services (District 12). You will play a key role in helping clients remain safely in their homes, improve their well-being, and access vital services and benefits.

Your work will be person-centered, culturally responsive, and rooted in building strong, trusting relationships with both clients and caregivers.

What You'll Do

  • Conduct client intakes, assessments, and re-assessments—both in-person and in-home.

  • Develop individualized care plans based on client needs and community resources.

  • Connect clients to services, benefits, and entitlements; advocate on their behalf.

  • Coordinate with caregivers and informal support networks.

  • Maintain accurate, up-to-date case records and documentation.

  • Collaborate with QCH colleagues and refer clients to other internal programs.

  • Assist in managing Supplemental Services Funds and outreach efforts as needed.

  • Participate in required trainings, staff development activities, and supervision.

  • Other duties as assigned.

What We're Looking For

  • Bachelor's degree required; MSW or related Master's degree (social services, public health, public administration, nursing) strongly .

  • Minimum 2 years of experience working with older adults.

  • Cultural competence and sensitivity to the diverse populations we serve.

  • Familiarity with hearing and vision impairment communication needs.

  • Spanish or Bengali fluency highly .

  • Strong interpersonal, organizational, and problem-solving skills.

  • Experience using client databases; knowledge of STARS is a plus.

  • High level of professionalism, attention to detail, and confidentiality.
