Case Manager 1- Housing - CREDO COMMUNITY CENTER : Job Details

SCHEDULE:Full-Time: Monday-Friday 8:00 am-4:30 pm. SUMMARY - Promotes the ability of high-risk individuals in the community-based housing programs to achieve independent living skills and stability in their psychiatric or medical conditions. Case manager works with individual to maintain housing and prevent homelessness. This includes tenant advocacy, apartment inspections, and finding new apartments as needed. Case/care management links individuals to service systems, then coordinates and monitors the provision of services. Services are individualized to the specific identified needs of each person in a culturally sensitive way.
