Your Role and Responsibilities

  • Collaborate with an interdisciplinary team to design treatment plans tailored to patient needs.
  • Establish tentative discharge plans in coordination with the team.
  • Participate in the execution of patient discharge experiences and monitor satisfaction metrics.
  • Facilitate weekly team conferences, ensuring all treatment plans are regularly assessed and modified as necessary.
  • Complete necessary documentation and case management reports, maintaining compliance with regulations and policies.
  • Ensure timely communication with patients and families regarding care and expectations.
  • Assist patients with resource procurement to prevent discharge delays.
  • Coordinate with external providers for post-discharge referrals.
  • Participate in utilization review processes to evaluate care trends and inform future planning.

Qualifications

  • License or Certification:
    • Must be qualified to conduct assessments independently within the scope of practice for your discipline (e.g., RN, SW, OT, PT).
    • If licensure is required, you must maintain an active license in the relevant state.
    • Eligibility for CCM® or ACM certification must be established upon entry, and certification must be obtained within two years.
  • Minimum Qualifications:
    • For Nursing roles, an Associate Degree in Nursing is required, with RN licensure; a BSN is preferred.
    • For other healthcare professionals, a minimum of a bachelor's degree is required; a graduate degree is preferable.
    • A minimum of two years of rehabilitation experience is preferred.
