Your Role as a Case Management Specialist

  • Collaborate with an interdisciplinary team to create treatment plans tailored to patient preferences and needs.
  • Coordinate discharge plans, including contingency strategies, in partnership with your team.
  • Manage patient experiences throughout their stay, ensuring quality and timely services.
  • Lead and document weekly team conferences, making necessary adjustments to treatment plans.
  • Conduct regular assessments and complete required documentation for case management.
  • Stay informed about regulations and company policies to ensure compliance.
  • Analyze case management reports to inform actions for improvement.
  • Engage in meetings to identify trends and mitigate issues related to acute care transfers.
  • Meet with patients and families within 24 hours of admission to establish communication and support.
  • Provide education on rehabilitation and the role of Case Managers to patients and families.
  • Schedule family conferences as needed to facilitate patient support.
  • Assist in resource procurement to prevent discharge delays.
  • Monitor compliance regarding orthotics and prosthetics orders.
  • Ensure accurate documentation for post-discharge referrals.
  • Participate in utilization review processes to enhance care quality.
  • Join the on-call schedule for case management as required.

Qualifications

  • Licensure and Certification:
  • Hold an active license to perform assessments within your discipline (e.g., RN, SW, OT, PT, ST, and Rehabilitation Counseling).
  • Meet eligibility for CCM® or ACM certification upon entering this position, or within two years.
  • CCM® or ACM certification needed or must be obtained within two years of becoming a Case Manager II.
  • Minimum Qualifications:
  • If a nurse, possess an Associate Degree in Nursing; a BSN is preferred. A diploma is acceptable only in states requiring it for licensure.
  • All other health care professionals must have a minimum of a bachelor's degree; a graduate degree is preferred.
  • Prior rehabilitation experience of 2 years is preferred.
