Become the Case Management Specialist You Aspire to Be

  • Collaborate with an interdisciplinary team to guide treatment plans based on patient needs and preferences.
  • Coordinate with the team to create and adjust tentative discharge plans as necessary.
  • Participate in and execute the patient discharge experience.
  • Monitor patient experience to ensure quality and compliance across various parameters.
  • Facilitate weekly team meetings to coordinate treatment plan adjustments.
  • Complete all necessary case management documentation and addendums.
  • Stay informed about regulations, company policies, and departmental operations.
  • Analyze case management reports and strategize appropriate actions for improvement.
  • Understand contract specifics, payor requirements, and recertification protocols.
  • Attend Acute Care Transfer (ACT) meetings to identify trends and enhance practices.
  • Engage with patients/families within 24 hours of admission to establish a supportive communication plan.
  • Perform assessments and complete required documentation within set timeframes.
  • Educate patients and families about rehabilitation and the role of Case Managers.
  • Organize family conferences as needed for optimal patient communication.
  • Assist patients in securing resources promptly to prevent discharge delays.
  • Oversee compliance with relevant regulations regarding orthotic and prosthetic processes.
  • Make timely referrals and provide necessary documentation to post-discharge providers.
  • Ensure the accuracy of discharge and payor-related information within patient records.
  • Contribute to the utilization review process through data collection and trend analysis.
  • Participate in the case management on-call schedule when required.

Qualifications

  • License or Certification:
  • Must be able to independently complete assessments relevant to your discipline (e.g., RN, SW, OT, PT, ST, Rehabilitation Counseling).
  • If licensure is required by the state, an active license must be held.
  • Must meet eligibility criteria for CCM® or ACM certification upon joining or within two years. Obtaining CCM® or ACM certification is mandatory within two years of taking a Case Manager II role.

Minimum Qualifications:

    • For nursing roles, an Associate Degree in Nursing is required, with RN licensure; a BSN is preferred. A diploma is acceptable only in specific states.
    • Eligible licensed or certified healthcare professionals must possess at least a bachelor's degree, with a graduate degree preferred.
    • A minimum of 2 years of rehabilitation experience is preferred.
