Become the Case Manager you aspire to be:

  • Collaborate with an interdisciplinary team to develop treatment plans that reflect patient needs and preferences.
  • Coordinate with the team to establish a tentative discharge plan and contingency strategies.
  • Engage in planning and executing the patient discharge process.
  • Monitor patient experiences concerning quality, timeliness, service appropriateness, payors, and expectations.
  • Facilitate weekly team meetings and coordinate treatment plan adjustments.
  • Complete necessary case management documentation and addendums.
  • Stay updated on regulations, company policies, and departmental operations.
  • Analyze case management reports and implement corrective actions as needed.
  • Understand commercial contract levels, exclusions, and payor requirements.
  • Attend Acute Care Transfer meetings to identify trends for improving outcomes.
  • Meet with patients and families according to established standards within 24 hours of admission.
  • Assess goals and complete case management paperwork within 48 hours of admission.
  • Educate patients and families about rehabilitation and the role of the Case Manager.
  • Organize and conduct family conferences as needed.
  • Assist patients with timely resource planning to avoid discharge delays.
  • Ensure compliance with ordering and payment regulations for orthotics and prosthetics.
  • Make timely referrals and maintain accurate documentation for post-discharge providers.
  • Verify the accuracy of discharge information in patient records.
  • Participate in utilization review processes, including data collection and trend analysis.
  • Engage in the Case Management on-call schedule as necessary.

Qualifications

  • License or Certification:
  • Must independently complete assessments within the discipline's scope (e.g., RN, SW, OT, PT, ST, and Rehabilitation Counseling).
  • If applicable, must hold an active license required for practice in the state.
  • Must meet eligibility for CCM® or ACM certification upon joining or within two years.
  • CCM® or ACM certification required or must be obtained within two years in the Case Manager II role.

Minimum Qualifications:

  • Nursing candidates must have at least an Associate Degree in Nursing; RN licensure is required, with a BSN preferred. A diploma is acceptable in states where it is the minimum requirement for licensure.
  • All other licensed or certified health care professionals must have a minimum bachelor's degree; a graduate degree is preferred.
  • Two years of rehabilitation experience is preferred.
