Become the Case Manager You've Always Wanted to Be
- Collaborate with an interdisciplinary team to guide treatment plans tailored to patient needs.
- Coordinate discharge plans and establish effective contingency plans.
- Contribute to optimal patient discharge experiences through careful planning and execution.
- Monitor patient quality and service satisfaction, ensuring care appropriateness.
- Facilitate weekly team conferences to discuss treatment modifications.
- Complete all required documentation, including case management addendums.
- Stay informed about regulations, policies, procedures, and departmental operations.
- Review and analyze case management reports for Key Care Indicators and implement appropriate actions.
- Understand commercial contract levels, payor requirements, and recertification processes.
- Identify trends in Acute Care Transfers during ACT meetings and work collaboratively to reduce them.
- Engage with patients and their families within 24 hours of admission to discuss rehabilitation and your role as their Case Manager.
- Assess patient goals and finalize necessary documentation within 48 hours of admission.
- Educate patients and families about rehabilitation processes and establish a clear communication plan.
- Arrange and lead family conferences as required.
- Assist patients in securing timely resources to prevent discharge delays.
- Ensure compliance with regulations for ordering orthotics and prosthetics.
- Make timely referrals and maintain accurate records for post-discharge follow-up.
- Participate in the utilization review process, including data collection and trend analysis.
- Be available for case management on-call duties as needed.
Qualifications
- License or Certification:
- Must be qualified to conduct assessments independently based on your clinical discipline (e.g., RN, SW, OT, PT, ST, Rehabilitation Counseling).
- Active licensure required if mandated by your discipline in your state.
- Eligibility for CCM® or ACM certification is required upon entry and must be obtained within two years.
- Must gain CCM® or ACM certification within two years of obtaining the Case Manager II position.
Minimum Qualifications:
- For Nursing: Associate Degree in Nursing required; RN licensure with BSN preferred. A diploma is acceptable only in states where it meets the minimum licensure requirements.
- For all other licensed or certified health care professionals: A minimum bachelor's degree is required, with a graduate degree preferred.
- Preferred: 2 years of rehabilitation experience.
