Case Management Opportunity

Interviews, accepts, and provides comprehensive, long-term, structured, complex case management services for an assigned caseload of clients participating in an established life management program; understands the uniqueness of the client's history to determine the most effective program plans; develops comprehensive program plan/goals and evaluates client's progress by conducting mentoring and counseling sessions with client and staff; serves as advocate for client to acquire services that will enable them to cope with their environment functionally; ensures constant compliance with funding requirements. Works with clients in the Veterans Outreach Program.

Key responsibilities:

  • Interviews, accepts, and provides comprehensive, long-term, structured, complex case management services for an assigned caseload of clients participating in an established life management program; understands the uniqueness of the client's history to determine the most effective program plans.
  • Assists clients in locating/securing affordable housing; works closely with clients to transition into the new housing; conducts home visitations when appropriate; ensures that client's living quarters are properly furnished and maintained.
  • Prepares and maintains case records and logs on all assigned clients; ensures the accuracy and completeness of the same; enters pertinent information into the established Homeless Management Information System (HMIS).

Physical Requirements and Working Conditions:

Work requires the ability to lift and carry and/or push/pull light (less than 25 lbs.) materials, supplies, and equipment. Light mobility: duties are usually performed by a combination of standing, walking, and primarily sitting. Work is usually performed in a normal office environment where there is little or no physical discomforts associated with changes in weather or discomforts associated with noise, dust, dirt, and the like. Work requires driving a vehicle daily with exposure to the outdoors are physical discomforts associated with changes in weather such as rain, cold, snow, and heat, or discomforts associated with noise, dust, dirt, and the like.

What We Are Looking For In You:

Education and Experience: Bachelor's degree from an accredited college or university in Social Work, Behavioral Science, or a related field, and Three years progressively responsible experience providing direct case management social services including accessing clients' needs and developing individual, comprehensive, long-term action plans for recovery utilizing a wide variety of resources, or any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.

Certifications: Valid State Drivers License
