CASE MANAGEMENT PROGRAM MANAGER JOB SUMMARY

Responsible for the ongoing operations of both the ECM (Enhanced Care Management) Program and CCHC's SDOH (Social Service Case Management Services), including integration of these services. Recognizes the importance of identifying and connecting participants to critical resources and services in order to increase quality of life and maintain stability in the least restrictive environment. Works closely with and reports to the Director of Social Work and BH Services, in developing and implementing best practices and performance improvement measures for these services. Responsible for financial and strategic management, data collection, and accurate reporting to all contracted managed care plans as well as CCHC executive staff. Understands diversity and health equity concepts and able to translate those concepts into on site staff and patient relationships.

CASE MANAGEMENT PROGRAM MANAGER ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Assumes full responsibility for the strategic development, operations and expansion of ECM and SDoH services.
  • Maintains budget stability for these programs
  • Establishes and maintains relationships with all funders and ensures compliance with all grant and contractual requirements.
  • Supervises all case management staff to ensure quality care; hires and trains new staff when relevant
  • Develops milestones for ECM for program and budget sustainability and works to meet those milestones
  • Collects and submits data and reports for ECM as required, in a timely manner
  • Works closely with Revenue Cycling Department to ensure accurate billing and revenue collection
  • Develops new initiatives to promote identification and resource connections for patients with SDoH challenges; highlights SDoH as a priority in patient care
  • Expands visibility of ECM and SDoH at all clinic sites
  • Provides marketing and outreach to develop and maintain collaborative relationships with community resources
  • Creates guidelines for documentation of all case management activities, including case management assessments and treatment goals; establishes caseload guidelines
  • Accepts referrals from BH and medical providers and supports assignment of patients for case management services
  • Demonstrates accountability and maintains professional boundaries through consistent engagement and communication with supervision
  • Accepts referrals for case management for patients at all clinics sites, and supervises both in person and telehealth services
  • Manages provider caseloads
  • Participates in grant development

Requirements

CASE MANAGEMENT PROGRAM MANAGER EDUCATION, TRAINING AND EXPERIENCE

  • Three years of direct experience in overseeing case management or related programs and supervising staff or those programs ; Bachelor's degree required, Master's degree in relevant field preferred
  • 1-3 years of experience in working in a health care setting; preference for candidates with experience working with managed care plans and MediCal
  • Demonstrated experience in documentation, and in utilizing Electronic Medical Records
  • Skilled in personnel management, supervision, and resource development

CASE MANAGEMENT PROGRAM MANAGER PAY RATE: $69k-$73k/yr
