Become the Case Manager You Aspire to Be

  • Collaborate with an interdisciplinary team to tailor treatment plans according to patient needs and preferences.
  • Work together to develop tentative discharge plans and contingency measures.
  • Engage in planning and executing patient discharge experiences.
  • Monitor various aspects of patient care, including quality, timeliness, and service appropriateness.
  • Facilitate weekly team conferences and coordinate treatment plan modifications.
  • Document case management addendums and fulfill all documentation requirements.
  • Stay informed on regulations, company policies, and department operations.
  • Review case management reports and develop actionable strategies for improvement.
  • Understand payor requirements and assist with recertification needs.
  • Participate in Acute Care Transfer meetings to analyze trends and identify improvement opportunities.
  • Meet with patients and families as per established standards post-admission.
  • Perform goal assessments and complete necessary documentation within specified timeframes.
  • Educate patients and families about rehabilitation and your role as Case Manager; create communication plans.
  • Organize and lead family conferences as required.
  • Assist patients in timely resource acquisition to prevent discharge delays.
  • Ensure compliance with regulations for orthotics and prosthetics ordering.
  • Make necessary referrals to post-discharge providers/physicians promptly.
  • Verify the accuracy of discharge and payor-related records.
  • Engage in utilization review processes, including data collection and trend analysis.
  • Contribute to on-call case management efforts as required.

Qualifications

  • License or Certification:
    • Must be capable of independently conducting assessments consistent with your professional discipline (e.g., RN, SW, OT, PT, ST, Rehabilitation Counseling).
    • Active state licensure required where applicable.
    • Must fulfill eligibility requirements for CCM® or ACM certification upon entering this position or within two years.
    • CCM® or ACM certification is required or must be achieved within two years of transitioning to the Case Manager II position.

Minimum Qualifications:

    • Nursing candidates must have at least an Associate Degree in Nursing, with RN licensure; a BSN is preferred. In states allowing for diploma qualifications, this is acceptable if aligned with licensure requirements.
    • All other eligible licensed or certified healthcare professionals need a minimum of a bachelor's degree; graduate degrees are preferred.
    • Two years of experience in rehabilitation is preferred.
