Case Management LPN - The Judge Group : Job Details

About the Role:

As an LVN Case Manager, you'll play a vital role in supporting members with chronic conditions such as Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), Diabetes, and more. You'll work closely with the supervising RN to monitor high-risk, non-delegated members and help ensure they receive the care and attention they need to manage their health effectively.

Top Responsibilities:

• The LVN CM will support the GCM CHF, COPD, Diabetes, and other chronic conditions.
• The LVN CM focuses on the non-delegated members who are identified as high risk.
• The LVN can contribute by gathering and recording of information, data, observing, and monitoring and reporting of changes in condition to the supervising RN.
• Conduct interviews to understand cause and effect, gather or review health history for clinical symptoms, and determine health needs.
