Posting Details Posting Summary Position Title Case Management Coordinator (Grant Funded) Job Category Administrative Staff Salary Range $67,013 Position Type Full Time Department Division on Women Job Summary Coordinates the implementation of high-quality programs at The ReLaunch Center at CCM and provides case management services for participants funded through the NJ Division on Women Displaced Homemaker grant. Conducts intake assessments, makes referrals to appropriate community agencies, and provides case management and follow up for individuals accepted into the program. In collaboration with Employment Specialist, ensures that each participant develops a job readiness plan while addressing and stabilizing financial and emotional stresses. Within grant guidelines, provides leadership in identifying community needs and designing quality programs and services to meet those needs. Responsible for assessing processes and services and working with staff to ensure continuous improvement. Assists with and is responsible for maintaining participant records and documentation of services. Attends community social service agency meetings. Provides leadership to The ReLaunch Center team, volunteers, interns, and within the division. Schedules, organizes and hosts workshops, including divorce workshops and financial literacy workshops. Ensures smooth functioning of the Center's operations by completing administrative tasks, including answering and responding to phone and email inquiries that come through the Center's main phone number and email address; maintaining communication and schedules for ReLaunch Center volunteers; scheduling appointments for participants to meet with volunteers. Maintain a constant awareness of funding sources appropriate for the program and seek out additional grant opportunities in collaboration with Director of Institutional Grants. Write grant proposals, develop budgets for funding streams, and adjust budgets as necessary. Qualifications - Education Bachelor's degree in social work, public administration or related. Qualifications - Experience Five (5) years experience conducting case management and assessment. Experience leading a small team also preferred. Grant management knowledge and experience preferred. Qualifications - Skills Knowledge of human services Strong background working with women. Exceptional interpersonal, organizational, and communication skills. Demonstrated analytical and research skills. Experience in grant-funded positions or organizations. Knowledge of various social media and computer software programs, e.g., Microsoft Office Suite, Publishing Software, etc. Bi-lingual, Spanish preferred.
