Competency/Duty AreasJob KnowledgeDevelops, maintains and utilizes sound working knowledge of all program standards inaccordance with Vanderheyden Hall and OCFS policies and requirements.Demonstrates knowledge of OCFS and maintains contact with referral sources for updates onindividuals in care.Attends outside trainings pertaining to community resources for families and children.Attends court hearings, CSE meetings, intakes, service plan reviews, phase/clinical meetings, staff meetings, treatment team meetings, and other meetings as requested by supervisor.Quality of WorkProvides reports and evaluations in a professional manner within the time guidelinesestablished by OCFS, to include:Psychosocial assessments within 30 days of admission;Service Plan Reviews on a quarterly basis;FASPs on a 6-month basis or more as needed;Treatment team minutes on a weekly basis;Progress notes in Connections Build 18 System within 24 hours of any meetings with outside providers or residents.Behavior Support Plans and other Treatment PlansAttitudeUnderstands and appreciates differing points of view, expertise, and experience of other staff and administrators within and outside of Vanderheyden Hall;Understands the limitations and processes of systems both within and outside of Vanderheyden Hall;Works and relates effectively with all levels of administration and case managers and helping professionals inside and outside the organization;Works and relates effectively with SILP treatment TeamDisplays appropriate energy and enthusiasm within the job elements and expectations.InitiativeSeeks to improve the facilitation of services to designated individuals.Seeks to improve the organization of records and systems in order to facilitate services;Provides support, assistance, and information outside of the designated job provisions and description;Explores and researches new information and processes in the facilitation and provision of case management and services to and for individuals.DependabilityArrives to work when scheduled and attends all scheduled meetings.Informs supervisor of schedule on a weekly basis;Attends outside meetings pertaining to residents in care;Attentively supervise family visits with residentsInterpersonal relationsActively participates as a member of at least one committee within Vanderheyden HallGives and receives feedback and constructive criticism in a professional manner when neededMaintains a professional relationship with co-workers of Vanderheyden HallTeamwork/collaborationWorks closely with Treatment TeamMaintains a positive relationship with DSS case workers, teachers, school social workers, and others involved with the treatment of individuals.Planning and organizational skillsPlans services for the residence in a way that coincides with treatment plans and meets appropriate timelines.Files all completed documentation on individuals in clinical files.Registers individuals for school upon admissionAnticipates discharges and make appropriate referrals to outside agencies and within Vanderheyden HallSkills, Qualifications, Knowledge, Abilities and Physical Requirements:Minimum Education and/or Professional Qualifications/SkillsA Bachelor's degree and one-year experience working with youth and families and/or people with developmental disabilities and/or mental health issues.Completion of Connections Build 18 training; preferred.Valid NYS driver license with clean MVRSpecial Personal Skills, Qualities, Aptitudes and Physical RequirementsCompletion of Therapeutic Crisis Intervention training and the ability to perform hands-on if necessary.Ability to assist individuals in times of crisis.Ability to be flexible and multi-tasking.
