The Case Aide provides support and client related services within the Preventive Services Dept including support activities such as interviewing clients; collecting information about family situations; directly assisting children and family members in various ways, i.e. providing transportation, home visits, accompanying parents to service providers, participating in and reporting observations and findings at case conferences. Furthermore, Case Aide will carry administrative functions such as petty cash, reconciling receipts and generating reports.

  • Work closely with the Case Planners to provide collateral services to individuals and families. Attend JHV with Case Planners and CPS at initiation of case.
  • To assist and/or model for our parents (caretakers) and children methods to achieve their service plan goals. To participate in supportive services such as recreation and educational programs which support behavioral goals for the children.
  • To request all school and medical reports; document in Connections utilizing appropriate progress note template and file in case record. Place printed copies of all important document in case files.
  • To assist the families in accessing in-house and community-based services to address issues that place a child at risk of foster care placement and promote maximum safety, supervision and support for children and their families. This includes but is not limited to accompanying parents to programs, other service providers, domestic violence shelters, and any concrete services that may be needed. Complete referral applications and gather all required support documentation.
  • To enter their progress notes in CONNX within 5 days of the occurrence utilizing progress note template. Submit hard copies of their progress notes in draft form on a weekly basis to their supervisor. Insert copies of all printed progress notes in case file and file in cabinets. Quality of progress notes must be informative, descriptive and purpose driven.
  • To make required home visits including collateral caregiver homes where children are cared for during the month and collateral contacts, in order to meet ACS minimum contact mandates, as well as, requirements of the specific model assigned to. Always ensuring the safety of the children and family. Submit weekly calendar of scheduled visits to their supervisor.
  • Submit the monthly PROMIS report; and generate the case work contact reports, supervisor reports, characteristics reports as well as other necessary reports.
  • Participate in mandatory once a week individual and unit supervision; attend all mandatory trainings, general staff meetings, as well as serve on committees as assigned.
  • Check files for necessary updated documentation and assessments to ensure they are within the ACS, OCFS, and Rising Ground Standards.
  • Archive all closed cases by Logging, storing, shipping closed cases.
  • Provide ongoing coverage to the receptionist area as assigned in a rotation schedule.
  • Other duties as assigned.
  • Bachelors degree with a minimum of two years of prior experience as an office assistant preferred.
  • High School Degree or GED equivalent also considered with prior experience in child welfare environment required.
  • Must demonstrate strong organizational and computer skills including proficiency with Microsoft Word and Outlook. Familiarity with Excel a plus.
  • Drivers license a plus.
  • Bi-lingual (English and Spanish) strongly preferred.

In-office, with one (1) remote day every two (2) weeks.

(35) hour work week.
