Hybrid position (predominantly remote)Shift: Day shiftSchedule: Full-time 40 hours per week, Monday-FridayJob Summary: The Care Navigator is responsible for functioning in an expanded role to facilitate care through interdisciplinary collaboration and care coordination. Responsible for communicating with patients and caregivers to ensure improved care transitions, enhancing patient-practitioner communication to identify opportunities for care continuity, encouraging the patient's self-management skills and understanding of the health care system to guide and empower patients to participate actively in their care plan. An ideal candidate for this position would be comfortable being responsible for communicating with patients and caregivers to ensure care transition, communication for care continuity, self-management and understanding healthcare system for patients to actively participate in their care plan. Tasks with also include maintaining knowledge of community resources, payer requirements and network services, as well as serving as a patient advocate. This position may perform in-home visits Benefits:

Requirements Regulatory Requirements: High school graduate or equivalent. LPN/RN licensure in the State of IL preferred. At least three years' experience in a care coordination and/or case management position, preferably in a clinic environment, or extensive clinical knowledge in Chronic Disease Management. Skills: Proficient with Microsoft Word, Excel, and Outlook. Experience with electronic medical records.
