Care Manager - ICMS - Acenda Integrated Health : Job Details

Engages consumers at the point of hospitalization and participates in the discharge planning process to assist with the transition to the community Provides post-discharge follow-up to support the consumer with community integration Conducts assessments within correctional facilities and attends court hearings to advocate for persons served. Provides supportive case management services in the community Works as part of a TCM team in providing services to persons in the community Completion of all required documentation as per program standards Maintain required levels of service per program standards Requirements: Bachelor's Degree in Human Services or Psychology.
