Position Title: Health Home Care Management Program Director Reports To: Chief Executive Officer Salary Range: $100,000 - $125,000 annually Position Overview: The Health Home Care Management Program Director is responsible for overseeing the comprehensive operations, development, and strategic growth of the Health Home Care Management program. This position provides leadership in program delivery, staff development, quality assurance, and regulatory compliance while ensuring the organization meets its mission of protecting and promoting the health, safety, and well-being of members receiving services. Primary Responsibilities: Program Leadership and Operations • Direct and oversee all aspects of the Health Home Care Management program, including strategic planning, implementation, and evaluation • Develop, implement, and monitor operational systems and processes to track program goals, progress, and outcomes • Ensure program compliance with all regulatory requirements, including Department of Health standards and Managed Care Organization protocols • Manage program resources to maximize efficiency while maintaining service quality • Create and implement strategic plans for program growth and development • Establish and maintain program policies and procedures in alignment with organizational goals Staff Management and Development • Provide direct supervision to program supervisors, and management staff • Oversee hiring, training, and professional development of program staff • Ensure all staff maintain appropriate trainings and continue to expand their knowledge base • Conduct regular performance evaluations and implement improvement plans as needed • Foster a positive, collaborative work environment that promotes staff retention and growth • Coordinate with Human Resources on personnel matters, including recruitment, retention, and disciplinary actions Quality Assurance and Compliance • Monitor and evaluate program effectiveness through data analysis and outcome measurements • Ensure compliance with all regulatory requirements and quality standards • Develop and implement quality improvement initiatives • Maintain accurate documentation and reporting systems • Prepare and submit required reports to regulatory bodies and executive leadership • Lead internal audits and program evaluations Stakeholder Relations and Communications • Serve as primary liaison with regulatory bodies, including Department of Health, Managed Care Organizations, and NY State Offices • Collaborate with internal departments including Quality Assurance, Billing, and Human Resources • Develop and maintain relationships with community partners and service providers • Represent the program at external meetings and professional forums • Manage communication with members, families, and stakeholders Required Qualifications: • Bachelor's degree required; Master's degree in Healthcare Administration, Social Work, Public Health, or related field preferred • Minimum 5 years of management experience in healthcare or social services • Demonstrated experience in program development and strategic planning • Strong knowledge of healthcare regulations and quality standards • Excellence in staff management and development • Proven track record of program growth and operational efficiency • Superior organizational and time management skills • Outstanding written and verbal communication abilities Physical Requirements: • Ability to travel between program sites • Extended periods of computer work • Occasional evening and weekend availability as needed Benefits: • Comprehensive medical, dental, and vision insurance • FSA/Dependent Care/Transit Benefits • Paid Time Off and Holiday Schedule • Professional development opportunities Note: This job description is not intended to be all-inclusive.
