Job PurposeResponsible for supporting nursing standards of excellence with advanced professional education to deliver high-quality care. Serve as a management team member, working with all departments to oversee daily operations. Provide guidance and best practices to achieve organizational goals, improve experience, satisfaction, and retention. Ensure care coordination is sensitive to diverse backgrounds and disabilities.

Job Responsibilities

  • Develop and ensure effective personalized care plans to improve satisfaction and outcomes.
  • Facilitate IDT meetings, SDRs, and other meetings, including attestations and supporting Virtual IDT.
  • Manage on-call responsibilities during non-operational hours, including follow-ups and grievance initiation.
  • Generate necessary Clinical Delivery reports.
  • Represent the Community Health Nurse in IDT meetings as needed.
  • Allocate staffing and resources to ensure seamless site operations.
  • Coordinate care with IDT and other parties to meet health goals.
  • Review UAS assessments for accuracy.
  • Investigate incidents and ensure proper documentation.
  • Maintain complete and accurate participant documentation.
  • Review participant charts for quality and address deficiencies.
  • Provide care management services during care transitions.
  • Arrange for long-term care services as needed.
  • Monitor and evaluate care plan effectiveness and adjust as needed.
  • Ensure documentation compliance with standards and regulations.
  • Stay updated on health issues, guidelines, and standards.
  • Contribute to service and policy improvements.
  • Collaborate with HR on staffing, onboarding, and training.
  • Evaluate staff performance and conduct performance reviews.
  • Investigate and resolve employee grievances.
  • Serve as Director of Patient Services for LHCSA if assigned.
  • Perform other duties as assigned.
  • Weekly Hours: 37.5 Hours

    Schedule: 8:30 AM - 5:00 PM

    Education and Qualifications:- RN or BSN from an NLN-accredited nursing school or 5+ years healthcare experience.- NYSED-approved infection control coursework.- Case Management Certification preferred within 2 years.

    Experience:- 2+ years management in a home health or related setting.- Customer service experience.- Long-term care insurance experience beneficial.- 1+ year working with the elderly.- Supervisory experience required.

    License: Current NY RN license required.

    Language: Bilingual preferred.

    Physical Requirements: Must meet physical demands such as standing, lifting, walking, and visual and auditory skills as detailed.

    Salary Range: $120,000 - $125,000

    Seniority Level: Mid-Senior level
