Care Coordinator - Seneca/Hancock County - harbor : Job Details

Harbor is seeking a Care Coordinator who is responsible for comprehensive, multi-system care coordination activities for youth with complex needs and their families. Position is full-time, 40 hours per week, and candidates preferably lives in Seneca or Hancock Counties. Position is remote and in the community. Education/ Experience/ Other Requirements: At least 3 years of experience (or 2 years with an Associate or Bachelor degree; or 1 year with a Master degree) in providing community-based services and supports to children and youth and their families or caregivers in areas of children's behavioral health, child welfare, intellectual and developmental disabilities, juvenile justice, or related public sector human services or behavioral health care field. Have background and experience in one or more of the following areas of expertise: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development.
