Care Coordinator/On Call - Griswold Home Care : Job Details

Our Client Care Specialist/ After Hour On Call position helps our community live assured by coordinating care for clients with the right caregivers. They collaborate with the local team to ensure our clients receive the best possible care and serve as case managers for the clients ongoing needs. Trusted Relationships: Our commitment to honesty and transparency assures the connections we build stay strong. Ability to work a rotating schedule 2/3 days during the week and every other weekend. Responsible for answering calls and communicating with caregivers, prospects, and clients from 4 PM to 8 AM on weekdays and weekends, beginning at 4 PM on Friday and concluding at 8 AM on Monday.
