Care Coordinator - Licensed Chemical Dependency Counselor (LCDC) Role Responsibilities This position provides substance abuse assessment and counseling services to individuals with a mental illness with a co-occurring substance use/abuse diagnosis. Family members of the patient may also receive counseling/education regarding substance use and the interference with the care and stability of the patient's mental illness. The LCDC must be able to provide individual/family and small group chemical dependency counseling, have knowledge of community resources, and support sobriety and relapse prevention. This position is responsible for coordinating referrals to inpatient Rehabilitation programs and provides continuity of care upon patient's return to outpatient services. This position reports directly to the Clinic Manager or designee. The LCDC performs other duties as assigned by the Clinic Manager or designee. Must be able to work within a multi-disciplinary treatment team. Must have excellent engagement, time management and organizational skills. Routine hours are 8:00 AM to 5:00PM, Monday through Friday with flexibility for schedule changes to allow for appointments and group services outside of routine work hours. The LCDC is responsible and accountable for the adherence to and promotion of the Center compliance policy. Salary
