As our Care Coordinator (CC), you'll be a key part of that mission—connecting residents and their children to essential services, advocating for their needs, and helping them plan for long-term wellness and independence.

Your Role: Advocate, Navigator, and Compassionate Problem Solver

As the Care Coordinator, you'll serve as a bridge between residents and the wide network of medical, behavioral, legal, and recovery support available to them. From securing vital appointments to designing aftercare plans, your work will ensure that every woman—whether pregnant, postpartum, or otherwise—has access to the services she and her family need to thrive.

What You'll Achieve

Connect Women and Families to Critical Resources

  • Assist in scheduling and supporting attendance at key medical and behavioral health appointments including PCPs, OB/GYNs, pediatricians, psychiatric care, lactation support, and more.

  • Design and implement individualized aftercare plans, including referrals to outpatient services, mental health care, and recovery housing.

  • Collaborate with external providers to ensure continuity of care during and after the program.

Support Navigation Through Complex Systems

  • Guide residents through interactions with probation, parole, DCF, and other external agencies.

  • Assist residents in resolving outstanding legal matters.

  • Understand and adhere to mandated reporting procedures for suspected child abuse and neglect (M.G.L. Chapter 119, Section 51A).

Provide Holistic and Individualized Support

  • Offer consistent information, referral coordination, and follow-up for a wide range of resident needs.

  • Coordinate access to Medication-Assisted Treatment (MAT) and psychiatric care while in the program and during aftercare.

  • Provide transportation support, including PT-1 coordination, to ensure residents can attend essential services.

  • Respond to crises and make emergency referrals when necessary (e.g., detox, hospitals, DTA, aftercare placement).

Contribute to a Safe, Structured, and Healing Environment

  • Support daily operations by answering phones, greeting visitors, and assisting with admission documentation.

  • Collaborate with clinical and support staff to deliver trauma-informed, person-centered care.

  • Assist with milieu coverage and help maintain a safe, supportive environment for all residents.

  • Represent Sunrise Ridge in the community and reflect our values in every interaction.

What Sets You Apart Lived Experience & Genuine Empathy

  • You may bring lived experience in recovery and understand the courage it takes to seek and maintain help. (A minimum of one year of continuous sobriety is required if applicable.)

Skills That Make a Difference

  • Strong communicator with the ability to build trust and rapport with diverse individuals.

  • Organized, adaptable, and confident in navigating both healthcare and human service systems.

  • Comfortable using Microsoft Office and web-based tools for documentation and coordination.

Education & Experience

  • At least one year of case management experience in a substance use or mental health treatment setting.

  • Bachelor's degree preferred.

  • Valid driver's license required.

  • Must be able to pass a CORI and Adam Walsh/CPS background check.
