- Assist case managers with scheduling, documentation, and follow-up tasks.
- Maintain accurate patient records and ensure timely data entry into care management systems.
- Prepare and organize bundled rate information and service outlines for patient review.
- Track patient progress and flag delays or issues for case manager attention.
- Support internal reporting and quality assurance initiatives.
- Communicate with patients to clarify bundled rate options and included services.
- Provide guidance on accessing our clients network and understanding financial responsibilities.
- Answer patient inquiries and direct them to appropriate resources or personnel.
- Ensure patients feel supported and informed throughout their care experience.
- Liaise between patients, providers, employers, and health plans to facilitate smooth coordination.
- Work alongside case managers to ensure care plans are executed efficiently.
- Participate in team meetings to review patient cases and improve service delivery.
Qualifications
- Bachelor's degree in healthcare administration, business, or related field preferred.
- 1–3 years of experience in healthcare coordination, administrative support, or case management.
- Proficiency in electric health record systems.
- Ability to manage sensitive information with professionalism and discretion.
- Familiarity with bundled payment models and healthcare navigation is a plus.
Hourly Rate: $30+/hr. dependent upon experience and education.
