Our extensive array of community-based services are fully integrated, bilingual and culturally competent.POSITION OVERVIEW The Care Coordinator is responsible for assisting members and their families/caretakers to navigate the healthcare and social services system in a patient centered manner. The Care Coordinator is the key resource for health home enrollees and assists in all aspects of care on a bio psychosocial continuumKEY ESSENTIAL FUNCTIONSMeeting with members in their homes, physician offices, public libraries or other agreed upon public places to introduce program, perform intake assessments, risk assessment and appropriate consents.Develop and maintain a strong, effective, on-going working relationship with assigned members.Link members to all bio psychosocial resources determined to be necessary by the member and assigned care coordinator.Work directly with Care Manager (Supervisor), member, family/caregivers, and discharge planners to assist in transition planningAttend and participate in regularly scheduled MANDATORY meetings with supervisor and other Team membersAssist members in translation & interpretation servicesCoordinate referrals and follow ups from physician/provider offices to care teamsHelp clients in utilizing resources including scheduling appointments, and assisting with completion of applications for programs for which they may eligibleMotivate members to be active, engaged in their healthAssist members in system navigation i.e., hospitals, physician offices and other health entitiesBuild and maintain positive working relationships with the members, care teams and all providersREQUIREMENTSAssociate degree required; B.S or B.A. preferred.Minimum of one (1) year experience in a health services environment.Working background in behavioral health and substance abuse illness preferred. Knowledge of chronic health conditions preferred.Familiarity with primary care practice preferred.Computer literacy, including proficiency in Microsoft Word and use of online reference and information resources required.Excellent oral and written communication skills requiredVerifiable good driving record and reliable transportationBilingual/bicultural encouraged to applyWHY JOIN US?Acacia Network provides a comprehensive and competitive benefits package to our employees.
