Care Coordinator - Woods Services : Job Details

The Care Coordinator will ensure that individuals are linked to mental health, substance abuse, medical services, and services targeting social determinants of health as required by SDOH (Social Determinants of Health) screening. Collaborates with other programs that individuals are involved with to ensure appropriate care Demonstrates the knowledge and skills necessary to provide care appropriate to any age-related needs of the consumers served Must be able to assess and interpret data about the consumers status in order to identify each consumers needs and provide the appropriate care, including age-related care, needed by the consumer group Bachelors degree in Psychology, Social Work, or related field Experience working with the population preferred Must possess a valid drivers license and if duties require driving, be able to drive an agency owned vehicle About Legacy Treatment Services Legacy Treatment Services is a vibrant non-profit behavioral health organization that utiliz...Care Coordinator, Coordinator, Behavioral Health, Healthcare, Treatment
