They will assist in coordination between departments, clients, and family members while clients are in services; they will work with the treatment team, clients, and family members to create a discharge/aftercare plan; they will coordinate with the treatment team and community recovery residences on a step down from residential to DT/IOP.Duties:Participates in weekly recovery team meetings and provides pertinent updates on case management needs, and discharge planning progress. Facilitates primary non-clinical related communication with family members throughout treatment; helps link families with appropriate departments, resources, and staff as needed. Completes a discharge planning needs assessment for all clients within 2 weeks of admission. \Ensures that clients' level of care is accurately reflected in EHR, making appropriate changes and notifying staff.Provides case management support while clients are in services, including (but not limited to): completion of ROI's, completion of letters verifying treatment, non-clinical tasks and communication with legal entities involved in a client's treatment, requesting additional medical records for clients post-admission, and providing support to other departments in communicating with families as needed.Assists in the scheduling and coordination of medical and psychiatric appointments with community providers as directed by the GFWC and attending psychiatrist.Assists in the coordination of step down to community based sober living programs as a client moves to the PHP/IOP levels of care.Coordinates logistics for voluntary and involuntary psychiatric hospitalizations of current clients: contacts local hospitals, provides relevant clinical information and insurance information to secure a hospital bed for client in crisis; arranges transportation to hospital; keeps clinical team apprised of situation. Following the admission, continues to coordinate logistics and communication with the treatment team at the hospital; coordinates the discharge and return to Skyland Trail services following stabilization.Coordinates discharge planning for clients at the direction of the attending psychiatrist. Coordinates with members of the treatment team to determine the recommended level of ongoing care as well as recommendations for post discharge care needs, incorporating all clinically significant information into the discharge planning process. Meets with clients to discuss and develop an individualized discharge plan. Utilizes case management skills and advocacy to link clients to resources, services, programs, and providers in the community. Assists clients in scheduling appointments, completing intake calls, and providing needed clinical information and records.Assists in administration and completion of discharge outcomes data and customer satisfaction survey.Establishes and maintains professional contacts with community providers as referral sources.
