Care Coordinator Intake Specialist This full-time position is responsible for initiating engagement into OhioRISE care coordination through phone calls and written communication. This position provides clients with access to OhioRISE, enrolls in intake status, collects required proof of guardianship as needed, schedules appointments and works with community partners, and other staff to facilitate urgent scheduling needs. This position calls for a wide range of skills including excellent customer service, ability to engage and motivate, interpersonal communication (written & oral), crisis intervention, attention to detail and organization. Education/Experience/Other Requirements:
- Requires at least a high school diploma or GED. Associate or bachelor's degree in social work, psychology, or a related field preferred.
- LSW preferred.
- Minimum three years' experience in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field, providing community-based services to children and youth, their family, or caregivers.
- Have a background and experience in at least one of the following areas: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development.
- Must establish and maintain effective working relationships.
Primary Duties:
- Maintains responsibility for outreach and engagement of clients into the OhioRISE program.
- Documents all contacts provided and maintains records utilizing the electronic record system.
- Maintains client confidentiality and ensures release of information is managed in accordance with federal guidelines and agency policy/procedures.
