Essential Duties and Responsibilities: Identify and coordinate referrals to team members via the Electronic Medical Records (EMR) system. Receive clinic referrals from providers and staff via EMR or face-to-face clinic setting. Identify patients that need care gaps closed, i.e. Annual Wellness Visits, colonoscopies, mammograms, etc. and contact patients and work to close these gaps. Reports results and pertinent information to patients and healthcare team members. Identify participating patients in need of disease management and opportunities for preventative health interventions. Regular communication with emergency and inpatient departments to determine whether outreach is necessary for discharged patients. Assists with assessment by collection of basic data for assigned patients in the clinic or on the phone. Documents basic assessment and care/instructions given. Continually demonstrates professionalism and works cooperatively with others. Participates in opportunities that enhance personal and professional growth seeking out continuing education focused on skilled nursing regulations, care, and/or reimbursement. Practices and behaves in a professional, safe and ethical manner at all times. Serves as a patient advocate. Maintains friendly and courteous interpersonal communication skills and a positive service attitude. Participates in performance improvement and educational activities. Completes all CEU's as required by respective licensure. If licensed, maintains an active and current license. Works willingly and accepts assignments as given. Attends all mandatory meetings and programs. Participates in opportunities that enhance personal and professional growth. Performs other duties as necessary or assigned. Complies with the Corporate Compliance Policy and all laws, regulations, and Standards of Conduct relating to the position and agrees to report any suspected violations of law or Standards of Conduct. Complies with all applicable state and federal regulations and JCAHO standards. Maintains a safe work environment using safe work practices, utilizing standard precautions and complying with hazardous waste procedures at all times as described in St. Anthony's Safety Manual. Maintains the confidentiality of patient, resident, employee and organizational information.

Qualifications

Education and/or Experience: RN with state license required; Current license to practice nursing in the state. Bachelor's degree preferred. Family Service, Care Coordinator, or similar experience preferred. Position Requirements, Licenses, Certifications, Registrations: Basic Life Support certification. Excellent communication and interpersonal skills. Ability to build positive relationships with different types of people, including residents, patients, employees, and members of the health care team. Must possess effective time management skills. Ability to carry out assignments with minimal supervision and guidance. Knowledge of principles of anatomy and physiology, pharmacology and disease processes. Knowledge of medical laboratory techniques, principles, and instruments. Knowledge of medical office practice and procedures, office equipment, medical terminology, data entry, health insurance providers' rules and regulations. Language Skills: Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before customers, patients, families and employees of the organization.
