Care Coordinator - Bilingual Spanish (BK) - Soloh Partners : Job Details

Position: Care Coordinator Division: Mental Health Division Schedule: Monday - Friday 9am - 5pm (Hybrid 2 field days) Travel: 40%, (Travel Reimbursed) Hourly Rate: $23 Location Brooklyn (1 Monthly in-person visit to assigned caseload) Requirements:

• Bachelor's Degree in Human Services or related degree
• Minimum (2) years of experience providing direct services to clients suffering from mental health, substance abuse, or homelessness
• Bilingual Spanish Required

Responsibilities:

• Responsible for individual caseload of 25-30 adult clients, each of which is to be visited face-to-face once a month.
• Assure that clients attend medical appointments at outpatient clinics and comply with medication regimens.
• Helps participants obtain benefits including food stamps, ongoing financial and housing needs, health care
• Initiate appropriate referrals to additional services as needed.
• Facilitate and participate in client Case Conferences
• Immediately report serious incidents, allegations, and complete incident reports
• Write clear Progress notes of services and visits in the patient's electronic medical record, within 48 hours.
