Care Coordinator, Behavioral Health - Community Medical Centers : Job Details

Responsible for various aspects of care management to ensure that care being rendered is indicated, resources utilized are appropriate, and services provided are in the appropriate setting. Works collaboratively with the Case Management team, other departments and physicians to facilitate efficient and appropriate management of all cases. Qualifications Education. Bachelor's degree in Social Work, Psychology or related field. Experience. Two years acute Case Management experience within the past 5 years. PREFERRED: Case management experience to be in a field related to behavioral health.
